Correct Continuation Service Record Booklet - Where to find one ?

Probably a long shot but I'd like to get the correct period continuation service record booklet. Hoping someone may have a blank one or at least know where I may be able to obtain one. I have bought the latest service record booklet from Honda but it's not a continuation one and the latest style of book. I have all the service history receipts from Honda,for the cars 175000 mile life, so would like to add to the history with an up to date set of service books.

I've tried to post a photo of the ones I have but I'm struggling to upload one.
